Home Health Physical Therapist - Delco, NC

Deliver personalized home health physical therapy to patients in their homes; collaborate with physician and clinical team to develop, implement and revise the therapy treatment program; plan and administer prescribed skilled physical therapy treatment and training for patients to attain highest level of physical function; submit evaluation, treatment plans, progress reports and discharge summaries; provide supervision and written instructions to Physical Therapy Assistants and Home Health Aide staff; oversee and evaluate staff performance including documentation; report patient progress to physicians, Clinical Manager, staff, patient and family; drive to patient homes as part of care delivery; requires NC PT license, valid driver’s license, CPR certification; TB screening and driver-safety program participation; schedule includes 40 hours per week; sign-on bonus of $10K; competitive benefits package.
