Home Health Physical Therapist - Delco, NC

Confer with physicians and clinical teams to develop, implement, and revise therapy treatment programs for patients with injuries, illnesses, or functional disabilities. Administer prescribed skilled physical therapy treatment and training to help individuals regain strength, mobility, and independence. Submit evaluations, progress reports, and discharge summaries to supervisors and care management staff while providing written instructions to Physical Therapy Assistants and Home Health Aides. Oversee staff performance and monitor documentation completion. Report patient progress to physicians, clinical managers, and families. Requires a current North Carolina Physical Therapy license, valid driver's license, CPR certification, and 40 weekly hours.
