Job Summary
Plan and administer prescribed physical therapy treatment for patients in their homes, assess physical strengths to guide treatment goals, develop and implement rehabilitation plans aligned with physician plans, oversee PTAs and home health aides, document observations and evaluations promptly, and report patient progress to physicians and care teams. Requires collaboration with healthcare professionals and driving to patient homes; pay ranges from $93,000 to $128,000 annually with a 40-hour workweek, base pay, and pay-per-visit components, within Humana CenterWell Home Health. Must hold an active physical therapy license and CPR certification. TB screening and driver license requirements are part of the role.
