Job Summary
Provide in-home physical therapy to patients in the Phoenix metro area (Phoenix, Chandler, Tempe, Ahwatukee) to regain strength, mobility and independence. Plan and administer prescribed PT treatment in patients' homes; assess physical strengths to support physician evaluation; develop and implement rehabilitation programs aligned with physician Plan of Treatment and patient goals. Oversee PTAs and Home Health Aides with clear written care instructions and documentation; document patient observations, interventions and evaluations; report patient progress to physicians, Clinical Manager, care team, patient and family; maintain required evaluations, treatment plans, progress notes and discharge summaries. Role involves TB screening and driver/safety requirements; scheduled 40 hours per week; base pay ranges from $93,000 to $128,000 per year with competitive benefits.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
