Job Summary
Provide in-home physical therapy to patients to improve strength, mobility and independence; plan and administer prescribed PT treatments, assess and document patient function, develop rehabilitation programs aligned with physician plans, supervise PTAs and Home Health Aides with clear instructions, and communicate patient progress to physicians, care teams, patients and families. Includes scheduling 40-hour workweeks, TB screening as part of program, and driver-licensure requirements for home visits; compensation ranges per visit and per year with a strong focus on collaborative, patient-centered care.
Required Qualifications
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
