Home Health Physical Therapist

Evaluate functional status and develop individualized treatment plans for patients in their homes. Provide evidence-based therapeutic interventions to improve mobility, safety, and independence while educating patients and caregivers on exercises, equipment, and fall prevention. Coordinate closely with nurses, physicians, and other clinicians to ensure comprehensive care delivery.
