Job Summary
Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to guide treatment goals, then develop and implement a rehabilitation program consistent with the physician's Plan of Treatment. Provide clear written care instructions to Physical Therapist Assistants and Home Health Aides while overseeing their performance and documentation. Report patient progress to the physician, clinical manager, and care team, submitting all required evaluations, plans, progress notes, and discharge summaries promptly. This role supports CenterWell Home Health's mission to help patients regain independence and enjoy a better quality of life through personalized, comprehensive home care.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid state driver's license
- Personal vehicle liability insurance

- One year of experience as a Physical Therapist
