Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain highest levels of physical function. Assess and document patient strengths to guide treatment goals, then develop and implement rehabilitation programs consistent with physician plans. Provide clear written care instructions to PTAs and Home Health Aides, overseeing their performance and documentation. Report patient progress to the physician, clinical manager, and care team, submitting all required evaluations, plans, progress notes, and discharge summaries promptly. Maintain a valid state driver's license and personal vehicle liability insurance as part of the driver safety program.
