Home Health Physical Therapist

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain highest levels of physical function. Assess and document patient strengths to support physician evaluations and guide treatment goals, while developing rehabilitation programs consistent with the physician's Plan of Treatment. Provide Physical Therapist Assistants and Home Health Aides with written care instructions, oversee their performance and documentation, and report patient progress to the care team and family. Submit all required evaluations, plans, progress notes, and discharge summaries promptly. This role requires a valid state driver's license, personal vehicle liability insurance, and TB screening as part of the Humana driver safety and patient screening programs.
