Home Health Physical Therapist

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patient strengths to guide treatment goals, then develop and implement rehabilitation programs consistent with physician plans. Provide clear written care instructions to PTAs and Home Health Aides while overseeing their performance and documentation. Report patient progress to the physician and care team, submitting all required evaluations, plans, progress notes, and discharge summaries. This role is part of CenterWell Home Health's personalized home care program for patients recovering from injury, illness, or surgery.
