Job Summary
Plan and administer prescribed physical therapy treatment for patients in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluations and guide treatment goals. Develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and patient team objectives. Provide clear written care instructions to Physical Therapist Assistants and Home Health Aides while overseeing their performance and documentation. Report patient progress to the physician, Clinical Manager, and care team, submitting all required evaluations, plans, progress notes, and discharge summaries. This role supports patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization through personalized, comprehensive home care.
Required Qualifications
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid state driver's license
- Personal vehicle liability insurance
