Home Health Physical Therapist

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain highest levels of physical function. Assess and document patients' physical strengths to support physician evaluation and guide treatment goals, while developing rehabilitation programs consistent with the physician's Plan of Treatment. Provide PTAs and Home Health Aides with clear written care instructions, oversee their performance, and document patient observations, interventions, and evaluations promptly. Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries. Maintain a valid state driver's license and personal vehicle liability insurance as part of the driver safety program.
