Home Health Physical Therapist

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to guide treatment goals and develop rehabilitation programs consistent with the physician's Plan of Treatment. Provide written care instructions to PTAs and Home Health Aides, oversee their performance and documentation, and report patient progress to the care team and family. Submit required evaluations, plans, progress notes, and discharge summaries promptly. This role supports patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization as part of CenterWell Home Health's personalized care teams.
