Evaluate functional status, develop and adjust individualized treatment plans, and provide evidence-based therapeutic interventions to improve mobility, safety, and independence. Educate patients and caregivers on exercises, equipment, and fall prevention while coordinating closely with nurses, physicians, and other clinicians.
