Home Health Physical Therapist

Home Health Physical Therapist at CenterWell Home Health (Humana) delivers in-home physical therapy to help patients regain strength, mobility and independence. Responsibilities include planning and administering prescribed PT in patients' homes, assessing and documenting physical strengths to guide treatment goals, developing and implementing a rehabilitation program aligned with physician plans, overseeing PTAs and Home Health Aides with clear written care instructions, documenting observations and progress, and communicating progress to physicians, the care team, patients, and families. Requires a physical therapy license, CPR certification, TB screening participation, and a valid driver’s license with vehicle insurance.
