Home Health Physical Therapist

Conduct in-home physical therapy evaluations, create personalized care plans, and provide evidence-based treatments to improve mobility, safety, and independence for seniors and adults with chronic conditions. Educate patients and caregivers on care strategies, document all interactions in electronic medical records, and collaborate with an interdisciplinary team to coordinate patient needs.
