Home Health Physical Therapist

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patient strengths to guide treatment goals, then develop and implement rehabilitation programs consistent with the physician's Plan of Treatment. Provide written care instructions to PTAs and Home Health Aides, overseeing their performance and documentation while reporting progress to the care team and submitting required evaluations. This role involves working within CenterWell Home Health's personalized care teams to help patients recover from injury, illness, or surgery.
