Home Health Physical Therapist

Plan and administer prescribed physical therapy treatment for patients in their homes to attain highest levels of physical function. Assess and document patients' physical strengths to guide treatment goals, then develop and implement rehabilitation programs consistent with physician plans. Provide written care instructions to PTAs and Home Health Aides, overseeing their performance and documentation while reporting progress to the care team. Submit all required evaluations, plans, progress notes, and discharge summaries promptly. Maintain a valid state driver's license and personal vehicle liability insurance as part of our driver safety program.
