Home Health Physical Therapist

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document physical strengths to support physician evaluation, then develop and implement rehabilitation programs consistent with the physician's Plan of Treatment. Provide clear written care instructions to PTAs and Home Health Aides while overseeing their performance and documentation. Report patient progress to the physician and care team, submitting all required evaluations, plans, progress notes, and discharge summaries promptly. This role is part of CenterWell Home Health's personalized home care program for patients managing chronic conditions or recovering from injury.
