Job Summary
Provide in-home physical therapy to help patients regain strength, mobility and independence by delivering prescribed PT treatment, assessing patient function, developing and implementing rehabilitation plans, supervising PTAs and Home Health Aides, and documenting progress and outcomes. Communicate progress to the physician and care team, and ensure timely evaluations, plans, progress notes and discharge summaries. Requires current unrestricted PT license, CPR certification, and experience collaborating with a healthcare team; compensation includes pay per visit and annual base pay, with explicit TB screening and driver-licensing requirements.
Required Qualifications
