Job Summary
Provide in-home physical therapy to patients, plan and administer prescribed PT treatment at patients’ homes, assess and document physical strengths to guide treatment goals, develop rehabilitation programs aligned with physician plans, supervise PTAs and Home Health Aides with clear instructions and oversee documentation, and communicate patient progress to physicians, care teams, patients, and families while submitting evaluations, plans, progress notes, and discharge summaries.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
