Job Summary
Provide in-home physical therapy to patients in the Phoenix metro area, delivering prescribed PT treatments to restore function and mobility. Assess patients’ physical strengths to guide goals, develop and implement rehab programs aligned with physician plans, supervise PTAs and Home Health Aides with clear care instructions, document interventions and progress, and communicate patient status to physicians, care teams, patients and families. Role emphasizes teamwork, patient-centered functional improvement, and adherence to TB/safety programs. Requires current PT license and CPR, with experience collaborating in healthcare teams and a preferred 1 year of PT experience. 40 hours/week with competitive pay ranging from $93k to $128k annually depending on location and experience.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
