Home Health Physical Therapist

Plan and administer prescribed physical therapy treatment onsite in patients' homes to attain highest levels of physical function. Assess and document physical strengths to guide treatment goals, develop rehabilitation programs consistent with physician plans, and provide written care instructions to PTAs and aides while overseeing their performance. Document patient observations, interventions, and evaluations promptly, report progress to the clinical team and families, and submit all required evaluations and discharge summaries. This role supports CenterWell Home Health's mission to help patients regain independence through personalized care for those managing chronic conditions or recovering from injury.
