Job Summary
Home Health Physical Therapist responsible for planning and administering prescribed PT treatment in patients' homes, assessing physical strengths, developing rehabilitation programs aligned with physician plans, guiding and overseeing PTAs and Home Health Aides, and documenting progress; role involves in-home visits, collaboration with care teams, and communicating progress to physicians, Clinical Manager, patients, and families. Requires degree from an accredited Physical Therapy program, active PT license, CPR certification, and teamwork experience; one year of PT experience preferred. Involves patient-facing duties, TB screening participation, valid state driver’s license, and personal vehicle insurance where applicable. Compensation includes annual base pay range of $93,000–$128,000 and pay-per-visit options; full-time schedule with benefits through Humana CenterWell Home Health.
Required Qualifications
- Degree from an accredited Physical Therapy Program
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
