Home Health Physical Therapist

Plan and administer prescribed physical therapy treatment onsite in patients' homes to attain highest levels of physical function. Assess and document patient strengths to guide treatment goals, then develop and implement rehabilitation programs consistent with physician plans. Provide written care instructions to PTAs and Home Health Aides, overseeing their performance and documentation. Report patient progress to the physician, clinical manager, care team, and families, submitting all required evaluations, plans, progress notes, and discharge summaries promptly. This role involves delivering personalized care from the comfort of homes to help individuals regain strength, mobility, and independence.
