Job Summary
Home Health Physical Therapist providing in-home care to patients, planning and administering prescribed physical therapy, assessing and documenting patient strengths, developing rehab programs aligned with the physician’s Plan of Treatment, supervising PTAs and Home Health Aides, documenting observations and evaluations, and reporting patient progress to the physician and care team. Role requires TB screening participation and a valid state driver’s license with personal vehicle insurance; 40-hour workweek with pay range based on location and experience.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
