Deliver in-home physical therapy to patients, assessing strengths and function, developing and implementing rehabilitation programs aligned with the physician's Plan of Treatment, supervising PTAs and Home Health Aides with clear written instructions, documenting patient observations and progress, and communicating updates to physicians, the Clinical Manager, the care team, patients, and families to support functional improvement and independence.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
