Job Summary
Provide in-home physical therapy to patients, planning and administering prescribed PT treatment to restore function and independence. Assess patients, document progress, develop and implement rehabilitation plans aligned with physician orders, supervise PTAs and Home Health Aides, and communicate patient progress to the physician, Clinical Manager, care team, patient, and family through timely documentation and discharge summaries. Coordinate care to support improved mobility, strength, and quality of life for home-based patients.
Required Qualifications
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Current and unrestricted Physical Therapy license
- Current CPR certification
