Job Summary
Deliver in-home physical therapy services to patients, including planning and administering prescribed treatment, assessing physical strengths, documenting progress, and guiding goals within the physician’s Plan of Treatment. Lead and supervise PTAs and Home Health Aides with clear written care instructions, ensure timely documentation and communications with physicians and care teams, and drive to patient homes to provide care while maintaining TB screening and driver-safety requirements. 40-hour work week with pay range $93,000-$128,000 annually; includes benefits and center-wide support.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification
