Job Summary
Deliver prescribed physical therapy in patients' homes, assess and document patient progress, develop and implement rehabilitation programs per physician Plan of Treatment, supervise PTAs and Home Health Aides with clear care instructions, and communicate progress to physicians and care teams. Schedule includes full-time home-visits with driver-required responsibilities; travel to patient homes, documentation of interventions and outcomes, and reporting discharge summaries and evaluations. Emphasizes patient-centered functional improvement and coordination with the care team to achieve optimal mobility and independence.
Required Qualifications
- Current and unrestricted Physical Therapy license
- Current CPR certification

- TB screening program participation
