Home Health Physical Therapist

Provide in-home physical therapy treatment to patients, plan and administer prescribed therapy to attain highest physical function, assess and document patients' strengths to guide treatment goals, develop and implement rehabilitation programs aligned with physician plans, supervise PTAs and Home Health Aides with clear written care instructions, document patient observations and progress, and communicate with physicians, clinical management, and family. Requires degree from an accredited Physical Therapy program, active Physical Therapy license, CPR certification, and collaboration within a healthcare team. Pay includes base and per-visit components with an emphasis on home-based care; TB screening and driving requirements are in place. This role is positioned to support patient rehabilitation and independence through home health services.
