Job Summary
The PT Case Manager provides and oversees home health services for clients in the community, traveling to clients’ homes for 1:1 care, collaborating with the client, family, and health-care team to develop goal-centered care plans, and supervising licensed clinicians and unlicensed personnel. Responsibilities include initial and periodic assessments, updating the home health plan of care every 60 days, ensuring safety and abuse prevention, maintaining documentation and compliance with state/payer requirements, educating clients and caregivers, coordinating care with family and community resources, and delivering direct care across varied ages and diagnoses while managing schedule and finances. The role is based in Blaine, MN with service coverage in Hinckley and surrounding areas; travel by automobile is required; office hours with a full-time schedule.
Required Qualifications
- Currently licensed as a Physical Therapist (PT) in the State of Minnesota
- License must not be revoked, suspended, or restricted
- Proof of negative TB testing (Mantoux, TB blood test, or chest x-ray)
- Access to a dependable automobile for travel between locations
- Valid driver’s license and current car insurance when using a personal vehicle
- U.S. Citizenship or evidence of valid Alien Work Permit
- Disclosure of any criminal history related to provision of health services; convicted individuals will not be hired
- Pass initial and ongoing background studies and screenings (e.g., Minnesota Department of Health, Office of Inspector General)
- Two or more years of experience in a health care facility or home care field
- Ability to work independently, adapt to change, manage multiple assignments, and prioritize duties
- Strong communication, interpersonal, and teaching skills
- Basic computer skills; experience with Electronic Medical Records and Microsoft Outlook preferred
- Current CPR certification highly recommended
