Home Health Physical Therapist

Conduct home visits to adult and geriatric clients in Framingham, Natick, Ashland, and surrounding towns. Perform diagnostic tests measuring mobility, range of motion, transfer status, balance, and muscle strength. Develop and implement individualized care plans featuring manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise care plans while collaborating with nursing staff. Educate clients and family members on rehabilitative activities to promote independent living. Accurately document observations and interventions using a touch pad tablet. Requires a current Massachusetts Physical Therapist license.
