Home Health Physical Therapist

Conduct home health visits to adult and geriatric clients in Waltham, Weston, Wayland, Needham, Newton, and surrounding towns. Perform diagnostic tests and measurements including mobility, range of motion, transfer status, balance, and muscle strength. Develop and implement individualized care plans featuring manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise care plans while collaborating with nursing staff in the multidisciplinary team. Educate clients and family members on rehabilitative activities to promote independent living. Accurately document all observations, interventions, and evaluations using a touch pad tablet. Requires a current Massachusetts Physical Therapist license.
