Job Summary
Conduct home health visits to clients in Calvert County, Maryland, performing diagnostic tests for mobility, strength, balance, and safety. Implement integumentary protective interventions and develop individualized care plans including manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise care plans while collaborating with nursing on multidisciplinary care management. Educate clients and family members on rehabilitative care and instruct them in activities promoting independent living. Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Requires a current MD Physical Therapist license and graduation from an accredited program.
Required Qualifications
- A current MD Physical Therapist - PT license
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
Desired Qualifications
- Solid computer skills
- prior experience with electronic medical records (EMR) preferred
