Job Summary
Provide skilled physical therapy services in patients' homes under the direction of a physical therapist, educate patients and caregivers, collaborate with an interdisciplinary team to optimize patient well-being, manage plan of care, discharge planning, and maintain timely documentation across visits (Mon-Fri) in designated Minnesota territories.
Required Qualifications
- Graduation from a Physical Therapist Assistant education program from an accredited university/program
- Current, unrestricted state license as a Physical Therapy Assistant in the state of practice
- Valid CPR
