Home Health Physical Therapist Assistant PTA Full Time

Assess, teach, and deliver therapy interventions directed by the physician within the home health plan of care. Collaborate with interdisciplinary team members, update patients and caregivers regarding the plan of care, and provide skilled therapy services to relieve pain or restore function. Teach patients and family members on safe use of assistive devices, educate on rehabilitation techniques, and plan for client discharge with clear instructions. Report observations of patient condition, reactions to treatments, and changes in physical or emotional status to the designated supervisor and physician on a timely basis. Record clinical progress notes and complete daily reports to ensure continuity of care and agency schedules. Maintain consistent attendance and submit all credentials in a timely manner.
