Job Summary
Provide skilled physical therapy services in home health per the physician's plan of care, including direct treatments, patient and caregiver education, progress documentation, discharge planning, and collaboration with interdisciplinary team; require PTA degree, current state license, CPR, and valid driver’s license, with documentation and compliance responsibilities and ability to coordinate care across Brookfield, New Berlin, Muskego, Franklin, Elm Grove, West Allis, and Oak Creek locations.
Required Qualifications
- Graduation from a Physical Therapist Assistant education program from an accredited university/program
- Current, unrestricted state license as a Physical Therapy Assistant in the state of practice
- Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures
- Valid CPR and State Drivers License
