Job Summary
Provide in-home physical therapy under the supervision of a licensed Physical Therapist. Administer the plan of care, assist patients with therapeutic exercises, gait training, balance activities, transfer training, and mobility programs. Monitor patient progress and report changes to the supervising Physical Therapist. Educate patients and caregivers on home exercise programs, safety techniques, fall prevention, and proper body mechanics. Maintain accurate, timely EMR documentation. Collaborate with physicians, nurses, therapists, caregivers, and other healthcare professionals to coordinate patient care. Support patients in achieving maximum functional independence and quality of life. Adhere to federal, state, agency, and professional standards including APTA guidelines and state licensing requirements. Demonstrate cultural sensitivity and provide patient-centered care. Participate in case conferences, team meetings, and ongoing professional development activities.
Required Qualifications
- Associate degree from an accredited Physical Therapist Assistant program
- Current and unrestricted Nevada Physical Therapist Assistant license
- Current CPR/BLS certification
- Minimum of one year of PTA experience, preferably in home health, outpatient, skilled nursing, or rehabilitation settings
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to travel throughout assigned service areas
- Strong communication, organizational, and time-management skills
- Proficiency with electronic medical records (EMR) systems
