Home Health Physical Therapist Assistant (PRN)-East Dallas, TX

Deliver physical therapy services in the home setting under the supervision of a licensed Physical Therapist and in accordance with the physician's plan of care. Assist in identifying patient goals, implement interventions, and develop individualized care plans using evidence-based treatments to improve mobility and functional independence. Educate and motivate patients to actively participate in their rehabilitation while accurately documenting all services and maintaining timely patient records. Monitor progress, evaluate outcomes, and communicate updates to the care team to ensure coordinated, high-quality care.
