Home Health Physical Therapist Assistant-PRN

Administer the physical therapy treatment plan to patients in their homes as part of CenterWell Home Health, performing non-complex active and passive manual exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities. Document client records and submit clinical documentation weekly, attend educational meetings, and coordinate department in-services. Notify Physical Therapist or Branch Director of equipment failures, supplies shortages, or staff/client changes. Apply infection control and universal precautions to patient care activities and use skills to impact patient independence and quality of life in a home-health setting.
