Job Summary
Deliver physical therapy services in the home setting under the supervision of a licensed Physical Therapist and in accordance with the physician’s plan of care. Develop and carry out individualized care plans, implement evidenced-based interventions to improve functional mobility and independence, educate and motivate patients, and collaborate with a multidisciplinary care team to ensure coordinated, high-quality care. Maintain patient records, monitor progress, and adjust treatment plans to support recovery and long-term well-being while promoting movement potential and reduced reliance on medications.
Required Qualifications
- Active Physical Therapist Assistant (PTA) license in Texas
- Current CPR certification
- Professional liability insurance coverage required
- Demonstrated clinical experience as a Physical Therapist Assistant
