Home Health Physical Therapist Assistant FT, Day

Provide physical therapy treatment under the direction of a licensed Physical Therapist, implementing and managing specific treatment plans while assessing patient response and modifying goals as indicated. Collaborate with team members to execute individualized care plans, participate in patient discharge planning, and obtain re-evaluations per the South Carolina Physical Therapy Practice Act. Deliver patient and family education regarding diagnoses, progress, and long-term functional outcomes, including referrals to community resources. Complete daily documentation of assessments, treatments, and progress in the computer system while adhering to departmental policies and chart review audit components. Maintain compliance with safety measures for patients and staff, hold a valid SC PTA license, and work a day shift at the Baptist Facility 1521 Home Health Midlands location.
