Job Summary
Provide in-home physical therapy services as part of a treatment team, administering the established PT plan, performing non-complex manual therapies and modalities, documenting care promptly, attending department in-services, and communicating equipment or staff needs; requires geriatric-focused knowledge, license, CPR, driving ability, and collaboration with clinicians to improve mobility and independence.
Required Qualifications
- Current state license to practice Physical Therapy
- Valid driver‘s license, auto insurance and reliable transportation
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
- Read, write, and speak fluent English
