Home Health Physical Therapist Assistant

Administer the physical therapy treatment plan to patients in their homes, including non-complex active and passive manual therapeutic exercises, massage, traction, heat, cold, water, and electrical modalities. Document client records and submit clinical documentation to the office weekly. Apply infection control and universal precautions while notifying the Physical Therapist or Branch Director of equipment failures, supply shortages, or staff/client changes. Attend educational meetings and coordinate department in-services. This role supports CenterWell Home Health's mission to help patients regain strength and independence through personalized care.
