Job Summary
Administer the physical therapy treatment plan to patients onsite in their homes, including non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, cold, water, and electrical modalities. Document client records and submit clinical documentation to the office weekly while applying infection control and universal precautions. Notify the Physical Therapist or Branch Director of equipment failures, supply shortages, or staff/client changes. Attend educational meetings and coordinate department in-services. Work onsite in patient homes under the direction of a Physical Therapist, requiring a current state license, valid driver's license, and CPR certification. This role supports CenterWell Home Health's mission to help patients regain strength and independence from the comfort of their homes.
Required Qualifications
- Current state license to practice Physical Therapy
- Valid driver's license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
- Read, write, and speak fluent English
- TB screening eligibility (part of Humana's Tuberculosis screening program)
- Valid state driver's license (per driver safety program)
- Personal vehicle liability insurance in accordance with residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
