Job Summary
Provide physical therapy services in patients' homes under the supervision of a Physical Therapist. Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, and modality treatments (traction, heat, cold, water, electrical) as prescribed. Document client records and submit clinical documentation weekly. Attend educational meetings and coordinate department in-services. Notify the Physical Therapist or Branch Director of equipment failures, supply issues, or staff/client changes. Apply infection control and universal precautions in patient care. Collaborate with an interdisciplinary team to support geriatric patients in regaining strength, mobility, and independence. This role involves patient-facing home visits with emphasis on functional improvement and quality of life.
Required Qualifications
- Current state license to practice Physical Therapy
- Valid driver's license, auto insurance and reliable transportation
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
- Read, write, and speak fluent English
