Job Summary
Administer physical therapy treatment plans to patients in their homes, including non-complex active and passive manual therapeutic exercises, massage, traction, heat, cold, water, and electrical modalities as developed by the Physical Therapist. Document client records and submit clinical documentation weekly, while applying infection control and universal precautions during patient care activities. Notify the Physical Therapist or Branch Director of equipment failures, supply shortages, or staff and client changes. Attend educational meetings and coordinate department in-services. Work onsite in patient homes with a valid state license, current CPR certification, and reliable transportation, reporting to the Clinical Manager within CenterWell Home Health.
Required Qualifications
- Current state license to practice Physical Therapy
- Valid driver's license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
- Read, write, and speak fluent English
- TB screening
