Job Summary
Administer physical therapy treatment plans, including non-complex active and passive manual therapeutic exercises, massage, traction, heat, cold, water, and electrical modalities to patients in their homes. Document client records and submit clinical documentation weekly while applying infection control and universal precautions. Notify the Physical Therapist or Branch Director regarding equipment failures, supply shortages, or staff/client changes. Attend educational meetings and coordinate department in-services. Maintain a valid state driver's license, auto insurance, and reliable transportation for onsite coverage in Syracuse, Solvay, Fairmount, and Onondaga Hill.
Required Qualifications
- Current state license to practice Physical Therapy
- Valid driver's license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
- Read, write, and speak fluent English
- TB screening eligibility (required for patient-facing role)
