Home Health Physical Therapist Assistant

Administer physical therapy treatment plans to patients in their homes, including non-complex active and passive manual exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities. Document client records and submit clinical documentation to the office weekly. Apply infection control and universal precautions while notifying the Physical Therapist or Branch Director of equipment failures, supply shortages, or staff/client changes. Attend educational meetings and coordinate department in-services. Maintain current state licensure, valid driver's license with insurance, and CPR certification. Work full-time (40 hours) as part of CenterWell Home Health's personalized care team, supporting patients recovering from injury, illness, or surgery to regain strength, mobility, and independence.
